Documents required for Lost/Stolen Passport

Choose this service if you cannot find your most recent passport or it was stolen, regardless of its expiration status.

Important to remember:

  • Fill out the DS-11 and DS-64 forms (Statement Regarding a Lost or Stolen Passport).
  • There’s no need to submit a police report.

Documents you’ll need at the time of processing:

DS-11 Application

Please follow these steps for the Online Government Passport Application Form DS-11

  1. Fill out the form online and print two single-sided copies.
  2. Note that hand-written forms will not be accepted.
  3. Make sure that the bar codes, labeled (DS-11 B), are present at the bottom right corner of both the first and second pages.
  4. Please provide (2) copies.

DS-64 Statement for Lost or Stolen Passport

  1. The DS-64 form must accompany the DS-11 form; please print two copies of each.
  2. You may complete the form by hand using only black ink or generate it on a computer and print it alongside the DS-11 form. Please refrain from electronically signing or submitting the form online.
  3. Fill out the DS-64, the Online Government Passport Application Form, as part of this process.

Passport Photo

To create a suitable passport photo, following measures should be taken in account.

  1. Color photo, size: 2 x 2 inches with White background
  2. Photo should be professionally taken and not old than 6 months.
  3. No uniforms, headgear, or white shirts.
  4. No Eyeglasses when taking photo.

  • Photo must in color. No grayscale photo is accepted.
  • Size must be 2(width) x 2(height) inches.
  • Printed on matte or glossy photo-quality paper.
  • Taken within the last 6 months, reflecting most recent appearance.
  • Full face, front view, head centered in the frame.
  • Neutral expression, eyes open, mouth closed.
  • Head size: between 1 inch and 1 3/8 inches from chin to top of the head.
  • Background: plain white or off-white, without distracting shadows on the face or background.
  • Uniforms are not recommended in photos, except for daily-worn religious attire.
  • Avoid hats or headgear that hide the hair or hairline.
  • Hearing devices, wigs, or similar articles typically worn should be kept on for the photo.
  • Glasses should not be worn; if medically necessary, provide a medical certificate.
  • Any articles worn for religious or medical reasons should include an explanatory statement.

Proof of Identification

  1. Include a copy (front and back) of a valid ID, issued more than 6 months ago. Make sure the copies are very clear and readable.
  2. Acceptable IDs include Driver’s License, State ID, Military ID, or any Government-issued ID.
  3. Paper IDs, Learner’s Permits, and temporary licenses are not accepted.

Proof of U.S. Citizenship

  1. Original or certified copy of U.S. Birth Certificate required (must include both parents’ names)
    • To get a certified copy of your birth certificate, reach out to the vital records office in your birth state.
    • For those born in California or Texas, a “long form birth certificate” displaying parents’ names is required.
  2. Birth Certificate must have a raised or embossed seal
  3. Original Naturalization Certificate also acceptable
  4. Marriage Certificate or Divorce Decree needed if there’s a change in last name
  5. Expired Passport can be used as Proof of US Citizenship.

Government Fee

The US Department of State will only accept checks or money orders written out properly.

  1. Check/Money Order amounting $190 for Passport Book only, or $220 for both Passport Book and Card.
  2. Make the check payable to the “US Department of State.” Please write as written.
  3. Pre-printed name and address required in the upper left-hand corner of the check.
  4. Starter or temporary checks are not accepted.
  5. Amount should be written in complete form. For example, if you are filling a check amounting $220, the amount should be written as “Two hundred and Twenty and 00/100”
  6. Applicant’s full name and date of birth must be written in the memo section of the check or money order.

Proof of Travel

  1. Common Proof of Travel includes airline, cruise, and hotel reservations.
    • Electronic airline ticket or email confirmation of the airline ticket.
    • Airline reservation.
    • Travel agency reservation.
  2. Alternatively, a Business Letter on original company letterhead can be used instead of a travel itinerary. The letter should include:
    • Expected dates of business travel.
    • A concise description of the business activities to be conducted.
    • Original signature from a colleague within your organization, other than yourself.
    • A sample of letter is attached to be printed on company letterhead.

Authorization Letter

The Letter of Authorization permits passport hand couriers to represent you in submitting your passport application.

  • The United States Passport Agency mandates your signature on this letter to release your new or updated passport.
  • Print the letter and fill it out by hand using a blue pen.
  • Ensure the form is completed neatly and avoid using a marker for filling out the letter.
