New Passport Application (DS-11 form)
Please use the DS-11 form if:
You have never possessed a U.S. passport; or
Your passport has been lost, stolen, or damaged (also complete form DS-64)
Passport Renewal Application (DS-82 form)
Please use the DS-82 form if:
Your previous passport was issued when you were 16 or older.
Your previous passport was issued in the past 15 years.
You currently have your previous passport in your possession.
Lost, Stolen, Damaged Passport Application (DS-11, DS-64 forms)
Please use the both DS-11 & DS-82 forms if:
You lost your passport
Your passport is stolen
Your passport is valid but damaged
Child/Minor Passport Application (DS-11, DS-3053 forms)
Please use the both DS-11 & DS-3035 forms if:
If your child is younger than 15, please complete DS-11 form online.
Utilize the DS-3053 Statement of Consent for a Minor form if One of the parents or legal guardians is unable to be present at the acceptance appointment.
Name Change Application (DS-5504 or DS-82 Form)
If you’ve legally changed your name due to marriage, divorce, or a court order, you can update your passport name by following these guidelines:
If your current passport was issued within the last year, use Form DS-5504 to request a name change.
If your passport is older than one year, you must renew your passport to update your name.
